The Madison County Health Department - Home and Community Based Waiver program is funded by Medicaid to assist senior adults and those of any age with physical disabilities to help them live in their homes as independently as possible. Our program staff can assist with the waiver application process. Adults and children who may not qualify for other Medicaid services may still be eligible through the Waiver program. We provide Case Management services and Attendant Care services which includes; light housekeeping, personal care assistance, mobility assistance, shopping, laundry and other services. The HCBW program may also cover adult day health care cost, home delivered meals and the cost of some medical supplies such as dentures, glasses, hearing aides, etc. Phone: (859) 623-3441 Fax: 859-626-4293
